GLP-1 FAQ's
How Do GLP-1's Work?
What is a branded GLP-1 medication?
Branded GLP-1s, renowned for their groundbreaking applications in diabetes management, have emerged as a promising and innovative solution for individuals seeking effective and sustainable weight loss. Engineered by leading pharmaceutical companies, these GLP-1 receptor agonists harness the power of Glucagon-Like Peptide-1 to not only regulate blood sugar levels but also to address the intricate interplay between metabolism and body weight.
The distinct feature that sets these branded GLP-1s apart in the realm of weight management is their ability to influence satiety and reduce appetite. The drugs work by mimicking GLP-1, a gut hormone produced naturally by your body that is involved in insulin release and appetite regulation. GLP-1 medications have been shown to encourage insulin release after you eat, while also inhibiting glucagon production and slowing down digestion. Together, these effects help to control blood sugar, reduce appetite, and lead to weight loss.
What is a compounded GLP-1 medication?
Drug compounding is often regarded as the process of combining, mixing or altering ingredients to create a medication tailored to the needs of an individual patient. Compounding includes the combining of two or more drugs. Compounded drugs are not FDA-approved. That said -
We ensure that the ingredients used in the prescribed GLP-1 compounds are pharmaceutical grade and NOT research grade.
We ensure that the GLP-1 medication is compounded using its base form and NOT its salt form.
The compounding pharmacy conducts third-party quality testing to check for potency, sterility and the presence of toxins.
We ensure complete due diligence of the compounding pharmacy, including but not limited to accreditations, licensures, and manufacturing/storage/tracking processes.
What are the most common side effects of GLP-1?
GI side effects are most common when initiating GLP-1 therapy. Nausea, diarrhea, constipation and lack of appetite are among the most common. Your provider may be able to prescribe medication to help manage these side effects. Often these side effects resolve over time.
How does GLP-1 therapy work?
GLP-1 therapy can help with weight loss by making you feel full and reducing appetite. GLP-1 is a hormone that our bodies naturally produce. When we eat food, GLP-1 is released and tells our brains that we're full. With GLP-1 therapy, doctors give you a medicine called a GLP-1 agonist. This medicine works like the natural GLP-1 in your body. It tells your brain that you're full even if you haven't eaten as much food. This can help you eat less and make healthier food choices. When you eat less and make healthier choices, you can lose weight over time.
How is GLP-1 therapy administered?
There are two ways GLP-1 medications are administered: orally and via injection. The injectable method is currently more common and is administered with a small insulin needle. You can inject this below the level of the skin in your abdomen, the outside of your thigh, or the outside of your upper arm. Check out this helpful guide on giving a subcutaneous injection from the University of Michigan.

Can GLP-1 therapy be combined with other chronic medications?
Yes, GLP-1 therapy can be combined with many other chronic medications such as those used for diabetes, blood pressure and high cholesterol. That said, there are certain medications and conditions which are not a good fit for GLP-1 therapy. It is important to notify both your PCP and your weight management clinician of any medications added or removed so they can continue to follow your progress and make other medication adjustments as needed.
What lifestyle changes should I make while on GLP-1 therapy?
When you're on GLP-1 therapy, there are some simple lifestyle changes that can help you stay healthy and get the best results from the medicine. These include eating healthy foods, drinking plenty of water, listening to your healthcare provider, and sleeping well. Remember, GLP-1 therapy can be helpful, but it's just one part of taking care of yourself. Making healthy choices with food, exercise and sleep will make you feel strong and happy. And don't forget to have fun and enjoy life with your friends and family!
Are there any interactions with other medications or medical conditions?
Yes, there may be interactions with your current medications. This is why it is important to list ALL of your medications AND supplements for your weight loss provider to review. Additionally, it is important to notify your in-person healthcare providers of any GLP-1 added so they do not start you on a medication that is contraindicated while on GLP-1 therapy.
What can I expect from my initial level of medication in terms of weight loss?
Please note that the initial level of medication is usually intended to be a starter dose to help your body get used to to the mechanism and effects of the medicine in association with a balanced diet and lifestyle program. It is common to not see significant weight loss on this level, but it is important for the reasons mentioned above. At your subsequent visit, your provider will evaluate your progress and increase the level of your medication if appropriate.
How long will it take to notice weight loss?
Weight loss can vary among patients. Some notice weight loss right away, others can take one month or more. Consistency is important. Continue to take as directed and your provider will titrate your dose as needed to promote optimal weight loss tailored to your clinical needs. On average, 1-2 pounds per week (or 6-8 pounds per month) of weight loss is typically considered safe. That said, individual targets may vary and should be discussed with your weight management clinician. Please note that prior to every visit, you will need an updated weight, blood pressure and heart rate measurement.
Who Is Not Eligible For GLP-1 Therapy?
Age: < 18 years and > 74 years.
BP >= 140/90 ii. iii. HIV Type 2 diabetes (not on insulin)

Cancer (active diagnosis, active treatment, or in remission or cancer-free for less than 5 continuous years; does not apply to non-melanoma skin cancer that was considered cured via simple excision alone)
Severe gastrointestinal condition (including but not limited to gastroparesis, blockage, etc.)
Currently untreated/unmonitored opioid, alcohol, or substance use disorder/dependence
Uncontrolled diabetes.
Clarification on Depression and Hypothyroidism as obesity related comorbidities
High TSH levels that are below 10 or low TSH levels need additional consideration.
What Should You Expect With GLP-1?
Why haven't I lost weight on my medication?
Some patients will notice an immediate weight loss and others will require higher doses of medication to achieve weight loss. Continue to use your medication as prescribed and your provider will titrate as necessary to promote weight loss.
When can I expect my side effects to resolve?
Most people find that side effects decrease over time. If you are finding it hard to manage side effects, please mention this to your provider.
How can I maintain my weight loss?
Exercise 3-5 days a week, drink plenty of water and eat a well balanced diet. Smaller portion sizes can help reduce excess calorie consumption.
As far as healthy eating goes, patients are not required to follow a specific diet, but it is important that you implement a reduced-caloric diet. Here are some recommendations:
Decrease calories and work up to a ~500 kcal daily energy deficit.
Consider intermittent fasting (IF), including time-restricted eating (TRE) and intermittent calorie restriction (ICR).
Eating plans such as the DASH (Dietary Approaches to Stop Hypertension) diet, Therapeutic Lifestyle Changes diet, the Mediterranean Diet, or following the American Heart Association (AHA) 2021 Dietary Guidance to Improve Cardiovascular Health are all worth considering.
If you need assistance with meal planning or have special dietary needs, it may be helpful to seek guidance from a registered dietitian.
Include a wide variety of nutrient-dense foods, such as fruits, vegetables, lean meats, low-fat dairy, seafood, nuts, seeds, legumes and whole grains.
Replace foods that are high in cholesterol, saturated fat (animal fats) and trans fats (fried, processed foods) with foods that are high in monounsaturated and polyunsaturated fats like olive oil, nuts, seeds and avocado.
Minimize refined carbohydrates, sweets and sugar-sweetened beverages.
If you cannot eat at least 1,200 calories each day, a multivitamin may be needed to ensure you’re meeting your nutritional needs.
After you reach your weight loss goal, it’s important to continue a healthy eating pattern to avoid weight regain.
What happens if I miss more than two doses of my medication?
If you miss more than 2 weeks of a GLP-1 medication, we suggest reinitiating the dosing schedule to reduce the chance of GI side effects. For example, if you are on level 3 of Ozempic, but have missed 3 weeks due to shortage or being out of town, we would restart you on level 1.
What do I do if there is a shortage of my brand medication?
Due to high demand of GLP-1 medications, some branded medications may be out of stock or short on supply. If your prescribed dosage is out of stock, we may not be able to fill a higher dose of your medication. Options include having our team complete and submit a Prior Authorization on other GLP-1 medications that your insurance might cover, discussing starting a compound medication, or your provider refilling your current dose of medication if it is available.
What is a compound medication?
Compounding is the process of customizing a medication by combining, mixing, or adjusting ingredients to meet a patient’s unique needs. These formulations are not FDA-approved, but we carefully vet every compounding partner through rigorous due diligence to ensure quality and safety.
We ensure that the product being used is:
Pharma grade (not research grade).
Base form (not salt form).
Passed through third party quality / sterility testing.
Here is some more information on this directly from the FDA.
What happens when I reach my goal BMI?
Once you reach a BMI of 25, your weight management clinician will start to de-escalate your dose. Once your BMI is between 22-24, you should ideally be at the lowest dose of medication. If you start to regain weight, your weight management clinician can begin to escalate the dose. It is recommended that you continue living a healthy lifestyle to maintain your weight loss.
Do I have to have a follow up every month?
For GLP-1 medications, follow ups are recommended at 1 month intervals until you have reached a steady state, at which time you will meet with your provider every 3 months.
How do I administer the medication my provider ordered today?

Longevity FAQ's
What is Sermorelin?
Sermorelin is a synthetic version of a naturally occurring substance that causes release of growth hormones from the pituitary gland. Growth hormones are naturally produced by the pituitary gland and are necessary for growth in children.
Is sermorelin used for weight loss?
Sermorelin is also known to potentially help patients lose weight because HGH increases the production of insulin-like Growth Factor-1 (IGF-1). Higher levels of IGF-1 result in higher levels of metabolism therefore aiding in weight loss.
Does sermorelin make you look younger?
Sermorelin causes an increase in growth hormone and cell turnover, and this naturally improves the quality of your skin. Most patients see a reduction in wrinkles and fine lines, and overall tighter skin, and possibly longer eye lashes.
There is not a lot of research on the effects of growth hormone secretagogues on skin texture and thickness because most growth hormone research has focused on changes in body composition. Stimulating natural growth hormone release is expected to increase collagen production and skin cell growth. Since growth hormone is an anabolic hormone, it increases skin cell size, growth, and cellular differentiation. Growth hormone stimulates the release of growth factors that can increase collagen production, skin density, mechanical strength, and skin thickness. Sermorelin is expected to help your skin maintain a more youthful appearance. Sermorelin can also help skin repair post-injury or after surgery.
Is sermorelin a steroid?
Sermorelin is not a steroid or growth hormone. It's a bio-identical substance that stimulates the pituitary gland to make more growth hormone, which is basically a man-made version of the naturally occurring substance responsible for releasing growth hormone.
Does sermorelin burn fat?
Sermorelin therapy can reduce body fat by 5-8% over the course of a year. It works by stimulating the pituitary gland to release human growth hormone, which either binds to growth hormone receptors or is carried out by insulin growth factor 1 (IGF-1). Higher IGF-1 levels lead to increased muscle mass and bone density, plus less fat.
Aging may lower natural growth hormone levels, so sermorelin helps restore them.
What is B12-MIC?
Vitamin B12 (cobalamin) is an essential, water-soluble nutrient critical for red blood cell production, neurological function, DNA synthesis, and cellular metabolism. The injectable form—especially methylcobalamin—delivers B12 directly into your bloodstream or subcutaneous tissue for optimal absorption, bypassing the gut. This is particularly important since B12 absorption naturally declines with age, GI disorders, or certain medications. B12 is crucial for anyone focused on energy, cognition, and aging well.
What is NAD +?
Nicotinamide Adenine Dinucleotide is a form of Vitamin B3 (Niacin). This is an essential coenzyme that is found in the mitochondria (the “energy powerhouses”) of all living cells. It is heavily involved in DNA repair and cellular remodeling. It has been known to “turn off” those genes that accelerate degenerative aging and are essential for maintaining neurotransmitter levels for the proper functioning of the brain. It is key in cellular metabolism. The heart, brain and muscle tissue cells contain the highest concentration of NAD+. The more NAD+ present in cells, the more efficiently they function. As we age, our NAD+ levels tend to decline, making us less able to repair cellular damage. We begin to lose energy, appear older and have decreased brain function.
NAD+ plays a significant role in anti-aging. With NAD+ the molecule works rapidly to repair cells throughout the body and neurons in the brain. It serves as a catalyst in ultimate brain restoration which assists in overcoming depression, anxiety, and common mood disorders. Restoring and supplementing NAD+ in the body may improve physical appearance, health, and quality of life.

Addt'l Information on Sermorelin
How long should you use sermorelin?
Sermorelin therapy is best for three to six months, but can be taken for up to a year. It causes cellular changes in the body, which take time for the body to adjust and fix. While it's safe for most people and long-term use doesn't have serious side effects, you should always follow your doctor's instructions. People with active cancer are not advised to take sermorelin as it may stimulate cancer cell growth.
In order to prevent saturation and desensitization of the growth hormone receptors in the body, it is recommended to use sermorelin on a 5 days on/2 days off schedule.
Also, a holiday washout period recommended for 2 weeks every 6 months to prevent tolerance and decreased efficacy.
Where can I inject sermorelin?
Sermorelin can be injected under the skin in different places, such as the lower abdomen, outer thigh, or upper arms. Good procedures should be followed when giving medicine, such as wiping the bottle's cap with antiseptics before inserting a needle.
Does sermorelin affect testosterone?
Sermorelin helps with many bodily functions, such as decreasing body fat, increasing endurance and strength, increasing lean muscle mass, and improving sleep. However, there is no scientific evidence that it increases testosterone levels in men or women.
When should I take sermorelin?
Sermorelin should ideally be taken before bedtime, as this is when the body produces the most growth hormone and repairs tissue. Taking it during the day is also an option, but important to note daytime fatigue can be a side effect, although extremely rare. The dose should be administered by subcutaneous injection and you should rotate injection sites regularly.
How much sermorelin should I inject?
Sermorelin is only available by prescription, and the dosage you need can vary from person to person. Your doctor will tell you the right dose for you. Make sure to follow the prescription closely and do not use more or less than what is prescribed. Altering the doses without your doctor's advice can increase the risk of side effects or lead to sub-therapy, so it's best to stick to what your physician has told you.
Is sermorelin FDA-approved?
Sermorelin, under the trade name Geref, was approved by the US Food and Drug Administration in 1997. In 2008 it was withdrawn due to difficulties with manufacturing, not safety issues. There are currently new drug applications for sermorelin acetate injection.
Can females take sermorelin?

For this reason, it is crucial to avoid its use until more definitive studies are available. Please consult with your oncologist or healthcare provider for further guidance.
What to do if I bruise?
If you develop a bruise after an injection, fear not. Employ these helpful strategies to bring you quick relief:
If you feel pain immediately after injecting, apply gentle pressure on the site to help prevent the development of a bruise. Do not massage or rub the injection site, as this will make the bruise worse. Treat the affected area with ice. Do not inject the NAD+ supplement on or near the site until the bruise has healed completely. If the pain is significant, contact your doctor and medical team.
No one was home to receive my order. Will the NAD+ be okay unrefrigerated?
To put your mind at ease, NAD+ remains stable at room temperature for several weeks. It only begins to degrade after being exposed to temperatures above 30°C / 85°F for a few days. Even then, it is not harmful, though it may lose some efficacy.
For your convenience, we include a cool pack under the bottom carton to maintain optimal conditions in transit.
For optimal freshness, we recommend storing NAD+ in the refrigerator upon arrival and after opening, keeping it away from bright light and extreme heat.
Will the injection hurt?
The injection can sting sometimes & bruises are not uncommon. Try removing the NAD+ from the fridge for 30 minutes beforehand as it can sting more when it’s cold.
Try various injection sites and a new one each time and a 45-degree angle can help. We hear frequently from customers that 1 day it can sting and feel a little sore afterwards & other days it’s painless and sting-free.
Where is it best to inject?
The syringes provided are intended for subcutaneous injections, which involve administering the injection just under the skin into the fatty tissue.
We suggest using areas like the stomach, hips, or buttocks for these injections. To administer, pinch the skin tightly between your finger and thumb with one hand, and with the syringe in your other hand, inject at a 45-degree angle.
It’s important to rotate the injection site each time to avoid skin irritation. It’s normal to occasionally experience a bruise or a small bump at the injection site; simply make sure to inject in a different area for your next dose.
What time of day is best to inject?
We suggest mornings as you may feel energized and not sleepy initially after injecting.
Before exercise can also be a good time as it can improve stamina and endurance.
How do I open my NAD+ vial?
It doesn’t seem to have a lid.
Your vial of NAD+ arrives sealed with a metal top.
Before you begin your supplementation, you simply need to remove and discard the inner metal circle and then you are ready to insert the syringe easily into the centre.
Wipe the top with an antiseptic swab before each use and store the vial in the fridge for optimal freshness..
How do I get the last of the liquid out of the vial?
Tilt and swirl: Tilt the vial slightly so the liquid gathers in one corner. Gently swirl it to help gather the remaining liquid towards the opening.
Use of syringe: Make sure the needle is correctly positioned in the area where the liquid is pooled. You might need to tilt the vial as you draw up the liquid.
Tap the vial: Gently tap the sides of the vial to move the liquid towards the bottom or the syringe entry point. This can help consolidate the liquid and make it easier to draw.
Air bubble technique: Insert the syringe needle into the vial, then push a little air into the vial. This increases the pressure inside, which can help force the last of the liquid into the syringe when you invert the vial.
